Referrals from Health Care Providers or Any Other Source
If you are a physician, nurse, addiction counselor, case manager, social worker, or any health care professional, agency, or organization who is interested in referring your patient or client to us, please download and complete our Referral Form and fax to (905) 514-4000. You are also welcome to call us with any questions or to make a verbal referral.
Patients can also self-refer to our clinic by calling us at 1-888-388-RAAM (7226).
Wait times for new patients are minimal, and we offer the opportunity for patients to be assessed the same day. Appointments may also be scheduled for times that are convenient, and we also accept walk-ins during our clinic hours (new patients need to arrive at least one hour prior to clinic closing time).
